 Individuals who want to participate in league play need to e-mail the following information:

1. Name (male or female for Coed)
2. Phone Number/s and e-mail
3. Position (goalkeeper, defender, etc...)
4. Division interested in playing (1st choice, 2nd choice)
5. Willing to be team Captain?

-Cost is $75 per player
-Payable by Cash or check made out to Wolf’s INDOOR SOCCER
-Payment will be collected by the team captain and must be handed in by the first game in order to participate.
